Care Coordinators
Team of Care Coordinators based at the practice. Provides co-ordination and navigation through the health and care systems. Signposts patients to information. Makes referrals to services and other health and care professionals. Please enquire at reception for referal

Digital and Transformation Lead
Undertakes transformation and improvement projects in PCN, including the adoption and optimisation of digital tools
